- Well, that is rather unfortunate...if it did happen before it could be that you either have your pc infected with malware or if you are using some sort of CMS ( wordpress, joomla...) then someone might have exploited some vulnerability.

- It has happened before, and I had fixed it. I just wrote a very angry email to my webhosting provider. I think that the problem is due to their poor security settings. I hope this will be solved soon.
